Profhilo at Ästhetik Institut Berlin is a bio-remodelling injectable treatment using one of the highest concentrations of hyaluronic acid available in any injectable product — 64mg of HA per 2ml syringe, comprising both high-molecular-weight and low-molecular-weight hyaluronic acid thermally stabilised (NAHYCO technology) without chemical cross-linking. This formulation is fundamentally different from traditional dermal fillers: because it is not cross-linked with BDDE, it does not create discrete volumes or structural augmentation. Instead, it spreads evenly through the tissue planes, acting as a biological scaffold that stimulates fibroblasts to produce new type I and III collagen and elastin. The clinical protocol for Profhilo uses the BAP (Bio Aesthetic Points) technique — a defined map of five injection points on each side of the face corresponding to areas of greatest bio-aesthetic impact: the malar cheek, the mid-cheek, the preauricular zone, the jowl, and the mandibular border. Each 0.2ml bolus is injected superficially and then spreads via hydraulic dissection through the subdermal layer, covering a large area from a single injection point. The five-point protocol minimises the number of injection sites required to treat the full face while maximising the surface area of HA distribution. The biological effect of Profhilo is driven by both components of the NAHYCO complex. The high-molecular-weight HA provides sustained slow-release hydration and space-filling within the extracellular matrix. The low-molecular-weight HA acts as a biological stimulant, binding to CD44 receptors on fibroblasts and keratinocytes and activating signalling pathways that upregulate collagen synthesis, elastin deposition, and adiponectin production (supporting the skin's fat architecture). The result is improved skin quality — increased hydration, elasticity, and firmness — visible progressively over 4–8 weeks. The standard Profhilo protocol at Ästhetik Institut Berlin is two sessions spaced four weeks apart. Each 30-minute session involves cleaning the skin, marking the BAP points, and injecting each point in sequence. Minor swelling and small bumps at injection sites resolve within 24 hours as the product disperses. No downtime is required. Maintenance with a single session every 6 months is recommended to sustain the bio-remodelling effect as natural collagen turnover continues.
